The following are the industry guidelines in relation to use of Group Insurance Fact Find form by all life and non life Insurers:

1 Objective
 
The Group Insurance (GI) Fact Find form serves as a means for insurers to obtain certain basic information in order to underwrite and price policies in a professional and prudential manner, with a view to sustaining long-term integrity of the industry as a whole.

2 Scope of Application
 
The form is effective from 1 October 2001 and is applicable to all distribution channels for new sales / business of:
  • Insured and non-insured programmes
  • Group health and life (combined)
  • Group life
  • Group health
The form will not apply to standard off the shelf packaged products where the benefits and premium rates are fixed and are intended for the clients to buy off the rack.

3 Signatory Requirements
   
3.1 For direct sales, the client's signature is to be appended in the GI fact find form.
3.2 For sales made via intermediaries, the client's signature plus the intermediary's signature as witness, are to be appended.
   
4 Code of Conduct/Practice
   
4.1 The GI fact find form seeks to obtain a set of minimum information, as an industry (life and non-life) standard, to enable the evaluation of risks to be undertaken. Individual insurers may impose stricter standards, i.e. enhance on the questions asked in the form.
4.2 Insurers are allowed flexibility in terms of the format / layout of the GI fact find form, provided the substantive contents (i.e. the minimum information) are followed.
4.3 No insurer will quote on any application / proposal form received that is not accompanied by the GI fact find form, duly completed and signed off. Incomplete forms, or forms without the required signatures will not be accepted.
4.4 No holding insurer will refuse to provide the information requested in the GI fact find form, particularly the claims history.
